Claim Your Tournament Prize

Follow these easy steps.

1. Have you registered on Extremehunting.net yet? Register here



2. Double check the tournament results before proceeding. Click here



3. Please print and fill out the Declaration Form, Click Here. Please include: 1.) An answer to all listed questions; 2.) A Witness Signature; and 3.) Include a photocopy of an identification card verifying your name, birthday and address if available.
   *Call to request a Declaration Form be sent to your mailing address or faxed if you are unable to print.
   *If you are under the age of 18, you must have a legal guardian or parent’s permission to claim a prize.



4. Declaration Form and Photocopy of Identification must be sent via fax (847)427-1065 or regular mail to Sega Amusements USA, Inc. 800 Arthur Avenue, Elk Grove Village, IL 60007, ATTN: Event Promoter.
